STEM Lab Truck Update

Students are bringing home a Participant Release and Informed Consent Agreement that must be signed in order for them to participate in the STEM Lab Truck experience. I am attaching the form to this message in case it does not make it home and you are able to print and sign it. If printing is not an option, you are welcome to stop by the school to sign a copy in person.

Please turn in all signed forms by Monday if possible. Tuesday is acceptable, but forms must be turned in before your child’s scheduled time to go to the STEM truck. Students without a completed form at their scheduled time will not be able to participate.
